Club Career
Sporting
Although Moutinho was born in Barreiro, Setbal District, he was born and raised in Portimo, Algarve. When he was a young player for Portimonense SC, he had a lot of potentials, and at the age of 13, he joined Sporting Clube de Portugal in Lisbon to continue his football training.
At just 17 years old, Moutinho was called up to the main team by manager José Peseiro during the 2004–05 preseason. He later went back to the Paulo Bento-coached junior squad, though, and played with players like Miguel Veloso and Nani to help the team win the national championship that year.
Porto
With a transfer fee of €11 million (€1 million was paid for 50% of the rights to center-back Nuno André Coelho) and a five-year contract, Moutinho signed with FC Porto on July 3, 2010, Sporting would also receive 25% of any added value, provided it increased over the prior value.

Monaco
On May 24, 2013, it was revealed that Moutinho and James Rodrguez had both signed for AS Monaco FC for a reported combined sum of about €70 million (€25 million for Moutinho). Wolverhampton Wanderers
On July 24, 2018, Moutinho signed a two-year contract with Wolverhampton Wanderers, an English team that had just been promoted, for an unknown sum that was reportedly about £5 million. On August 11th, he played 85 minutes in a 2-2 home draw against Everton to make his Premier League debut. On September 22, during a 1-1 draw away to Manchester United, he scored his first goal of the tournament from just beyond the area with his weaker left foot.
At the completion of his debut season, Moutinho was selected by Wolverhampton supporters as Player of the Season. He participated in all 38 league games during that time and made an additional six appearances during their FA Cup run to the semi-final, their first since 1997–98.
